Summerville Family Health Team (SFHT) is a group of 37 family physicians at 5 sites who provide primary care services to approximately 45,000 patients in Mississauga and South West Etobicoke.

Dr. Casey Corkum obtained an honours degree in Kinesiology through the Health and Human Performance program prior to graduating from Dalhousie University Medical School. He then completed his Family Medicine Residency through the Department of Family and Community Medicine at the University of Toronto and was based at the Trillium Summerville Family Medicine Teaching Unit (FMTU). In addition to a busy family practice, Dr. Corkum has a part time Sports Medicine practice based out of The Sports Clinic at the University of Toronto Mississauga Campus. He also surgical assists part time as well as caring for complex continuing care inpatients at Trillium Health Partners. He is also the Family Health Organization Executive member for the FMTU. Dr. Corkum holds a Clinician Teacher faculty appointment through the University of Toronto and is actively involved in the education of resident physicians and medical students. He received the "New Teacher Award" in October of 2013 as well as the "Excellence in Clinical Teaching" award in April of 2014.

Dr. David Daien has been practicing family medicine in Toronto for over 15 years. As a former electrical engineer and an experienced medical informatics consultant, he has pursued his interests in the appropriate use of technology to improve the quality of health care delivery.

David is a founding member of the Board of Directors of the Summerville Family Health Team (FHT). He is the IT and Quality Lead of this 33 Physician FHT and is responsible for design, selection, implementation and ongoing operation of their multi-site, multi-disciplinary Electronic medical record (EMR) with full integration to a large regional hospital.

Dr. Jess Goodman obtained his medical degree in 1978 from the University of Calgary. During his thirty years as a dedicated family physician, Jess has held the position of Medical Advisor and attending physician for several long-term care facilities.

Jess joined Summerville Family Health Team in 2007 and since then has played an integral role providing exemplary primary health care to his patients. Jess currently serves on the Summerville Health Team Board of Directors.

Since 2010, Jess has been actively involved as the President of VitalSines Inc., a manufacturer of arterial pulse analysis systems for use by health professionals. He is currently working on a worn wireless personal biosensor system for continuous monitoring of heart/breathing rate, blood pressure, temperature and motion.

Dr. Ted Nemtean graduated from McGill medical school in 1980. Upon graduating, he completed a 2 year residency at McGill family practice and was Chief Resident in his 2nd year.

In 1983, Ted joined Apple Hills Medical Clinic where he continues to practice family medicine today. During his 28 years at Apple Hills, Ted has had the opportunity to pass on his clinical knowledge and skills to medical students and nurse practitioners.

Ted has been a member of the Summerville Family Health Team Board of Directors since 2008.

Throughout Ted’s career as a family physician, he has been active in moderating numerous CME talks. In 2010, he was a principal investigator for an atrial fibrillation study. Currently he is a principal investigator for a diabetes study.

Ted’s leisure pursuits include cycling, golfing and skiing.

 
Dr. Somersall, Lee-Anne

Since 2006, Dr. Lee-Anne Somersall has worked as a full-time family physician in Mississauga, at both Credit Valley and Trillium Hospitals.  She has also worked as a Medical Contributor at the Canadian Broadcast Corporation, making dozens of appearances on various news programs.

After joining the Harborn Group of the Summerville Family Health Team in 2009, she became an active member of the Board of Directors in 2011.

In her spare time, Lee-Anne enjoys snowboarding, baking, and designing residential interiors.

 

Dr. Tazkarji, Bachir

Dr. Bachir Tazkarji graduated from East Carolina University where he completed his family medicine training followed by a year of a fellowship in geriatrics medicine.

After graduating, Dr. Tazkarji continued to pursue an interest in geriatrics.

He currently conducts consults at Toronto Rehab for dementia patients.

He is also part of the Summerville Family Health Team Memory Clinic.

Dr. Tazkarji is a family physician and a full time teacher at the Summerville Family Medicine Teaching Unit (FMTU) where he has been the residency site director since 2008.  Bachir has been a member of the Summerville Family Health Team Board of Directors since 2009.

Dr. Tazkarji sits on several committees including: Primary Core, Medical Education, FMTU Steering and Residency Teachers Committee.

Dr. Tazkarji has a continued interest in medical writing and dementia research.

In his spare time, he enjoys travelling, spending time with family and table tennis.
